Before the HRA: Making the Most of Your HRA

As part of the HRA, participants complete a lifestyle questionnaire. There are no wrong answers. It is a way to gauge lifestyle behaviors and learn how to improve your overall well-being and prevent problems down the road. Questions include:

  • Tobacco usage
  • Diet/exercise habits
  • Seatbelt usage
  • Completion of regular health screenings
  • Family Health History

Before the HRA:

  • You should fast for 8-10 hours prior to your scheduled HRA. Drink only water.
  • Fill out the lifestyle questionnaire in advance of your scheduled appointment.
  • If doing the on-line HRA, complete and submit the form as directed.
  • If you are using the paper version, print out and complete the form and bring it with you.
  • Be honest when completing the questionnaire.
